This is the GameDay writeup:

I haven't actually listened to the commentary because I've been at this game the whole time. But my best guess is that the pitcher ran & picked to the ball & threw it somewhere into left center trying to get Meidroth out at 2nd.
Must have been a hell of an overthrow to second because the batter was awarded home as opposed to it going into the outfield and him having to run hard. Would have had to end up in the stands, under a tarp, something like that.

I was guessing that somebody in the dugout came out and touched the ball while it was still in play. Or maybe the pitcher threw his glove at the ball... is that automatic three bases?

But it remains a mystery.

Found it... from Reading Phillies team website:
Yorke reached first on a single, and later stole second. Yorke then scored thanks to another single from Chase Meidroth. Meidroth took second on the play due to a throwing error from first baseman Jhailyn Ortiz, and was then rewarded home after the ball was thrown into the dugout before time was called. This added to the Portland lead 9-2.
Kind of shitty for the 1B to get the error when the whole play resulted from the pitcher dogging it.

If you watch the umpire and catcher it definitely has to do with something happening with the ball along the first base line, and it's an automatic rules-based award. I really think the pitcher threw it out of play in annoyance (or maybe as suggested someone else in the dugout touched it, but I don't know if that's a two-base error).

Edit: whoops, typing while @Mugsy's Jock was posting! Have some Kyle Teel as an apology.

Yeah, this Priddy guy always posts their #s with their pictures for some reason. It's an unusual convention.
